Finally you cant format all captions on this site, becouse some captions in source formatted in same way with some headers.
Also a lot of captions has very bad nested relatet to closest image and does'nt has anything to identifying it, And as I say some headers in <em> has similar nesting and order with captions, relates to closest image. So, I say better better to leave them as they are, it is no sens mark only part of captions and ignore another part.

It is in the same paragraph with the image and it has distinct formatting style.
