It's a mistake in the source, they set image width and height to 1px (that's why you see no image above title in this IV ORIGINAL window), and it's causing IV to not set it as cover, so this could be a one-off case. There might be another edge cases like 2px, 3px, I don't think it make sense fixing all those one-off cases.
