Missing second subtitle

The second "subtitle", as you call it, is actually a paragraph describing featured photo. Sometimes this paragraph is quite large (i.e. http://www.mercurynews.com/2017/06/08/phil-collins-takes-nasty-fall-in-bathroom-suffers-head-gash/) and given the fact that cover image already has one caption, I see no reason in mixing it with image caption and the mentioned paragraph.
That's why I styled this paragraph as a blockquote at the start of the article, this way it should be understandable to people even if they don't see the cover image and the paragraph describing it in one screen.

The "second subtitle" is a caption. It's even encapsulated in the <figcaption> tags and thus should also show up in IV. If the website decides to add 200+ words to a figcaption, it's their decision.

Also i wasn't able to find a figcaption anywhere on the link you posted.

The checklist says "captions must be preserved" and this text is part of the caption.
