1)They have totally different urls - IV doesn't have a functionality to differ duplicate images; 2)the first one is defined as cover on the source page and the second one is defined as just a picture
So everything is okay

It is inadvisable to use meta images as covers, especially when the image from metatags duplicates an article photo.
