When the image appears as a cover on the source website.
There's a suitable image above the title/subtitle or DIRECTLY BELOW THE TITLE/SUBTITLE. - this is not optional.

And sometimes on other pages of this site first picture can be part of the sequence, so you can't extract it as a cover .
