Looks like <h1>s with nested <em>s or <i>s are used as pullquotes (or something like that) on this site. Now it looks like a regular header in your IV, but not on the site (because of italic font use). Some page examples using this technique (just to prove that isn't a unique case):

If they are marked as a header in the source, they should be headers in the IV. That's definitely not a pullquote, but just a header with <em> tag inside, no classes or styles to identify.
