If you look at the source of the image, you'll see that the cover image URL is http://www.wikihow.com/images/f/fd/Hack-a-Computer-Step-24-Version-3.jpg , whereas the duplicate image is http://pad2.whstatic.com/images/thumb/f/fd/Hack-a-Computer-Step-24-Version-3.jpg/aid409235-v4-728px-Hack-a-Computer-Step-24-Version-3.jpg .

Having a look at the source code, one will see that the image displayed as a cover is mentioned as the cover image in the metadata. These can obviously differ and since they're on different domains and the URL can vary, it becomes more difficult to search for duplicates. Also, duplicating the cover image has been wikiHow's choice. Their cover image can be seen when selecting articles on the index page and in share images.
