- In my opinion, supporting this page is optional.
This site belongs to an association of animal rights activists. There is a large section dedicated to their news that is well suited to the targets of the IV.
Then there are pages like these, dedicated to the association's initiatives. Each page has a unique layout and some of them are even dynamic. We see that they are not articles because there are neither the date of publication nor the author.
I'm not surprised that the other template creators are having a lot of trouble trying to support all these pages.

- One is title, while other is header of question answered next.
Title can be either copy of question (when there's only one), altered or to generic category/name of questions answered on page (that happens even for pages with single question).
For example pages with single header (question) that is different from title: https://www.lav.it/leggi/macellazione https://www.lav.it/leggi/caccia https://www.lav.it/leggi/soccorso-animali https://www.lav.it/leggi/traffico-di-cuccioli https://www.lav.it/leggi/spettacoli And so on
It's not my fault that this page title is set to same text as question on it.

- There's no "correct" place to put it since gallery is in sepparate block to left of article text.

Since it is out of article body I think it is fine to append it to the start of article (site has no no other images under headline or any covers), you've digged deep to find article where it produces arguable result — 2013.
Pick any recent article and see that it's absolutely logical to put these slideshows under title/as cover. For example three most recent articles: https://www.lav.it/news/veneto-legge-ammazzalupi https://www.lav.it/news/violenze-animali-lancio https://www.lav.it/news/elefante-fugge-milano
