Cover image is missing.

"It is advisable to use a cover image when possible:

When the image appears as a cover on the source website.
There's a suitable image above the title/subtitle or directly below the title/subtitle."

The slideshow should not be broken to create a cover:

"Several images appear in the article one after another. In this case, the template should not break the sequence by extracting one of them as a cover"

First image of the slideshow was extracted as cover. This is not allowed.

Gallery counter (1 / 10 etc) is not part of the image caption.

The page contains a form, so requires user interaction. IV should not be generated.
